N2  - A single-center mixed-methods quality improvement study was conducted at Prince Sultan Military Medical City. A multidisciplinary steering structure implemented phased specialty-based building transfers, centralized patient placement, staff communication and training, and real-time performance monitoring. Quantitative measures included flow failures, average length of stay, patient turnover ratio, time from bed request to assignment, and system downtime. Pre- and post-intervention trends were assessed descriptively using run charts and linear trend analysis. Qualitative data included staff, patient, and management feedback. The number of flow failures decreased from 107 in April 2025 and 109 in May 2025 to 7 in August, 5 in September, and 9 in October 2025. The percentage of patients placed in an incorrect area/ward decreased from 3% in April and May to 0% from August through October. Average length of stay decreased from 9.7 days in May to 6.2 days in September, with a median of 7.5 days, meeting the target of ≤8.1 days. The patient turnover ratio had a median of 0.99 and reached ≥1 in August and September. No reported system downtime events occurred during the documented building reallocation period. Executive leadership–driven specialty-based building reallocation was associated with substantial improvements in patient placement accuracy, length of stay, and operational coordination. The findings support structural alignment, multidisciplinary governance, and continuous performance monitoring as practical strategies for improving quality and safety without major infrastructure expansion.
